A Fugitive Head is both an enemy and an item. The Headhunt activity, which parallels Helltides in a way, is the center of Season of Witchcraft. There are new Tree of Whispers bounties, new enemies (Headrotten), and new dungeons (Roothold).
At a certain point, while merrily crushing the skulls of your enemies, you’ll find out that this particular skull is different from others — and this is actually a Fugitive Head enemy. It might be tougher than others, but what you get after killing it is the actual Fugitive Head item that you can exchange for Occult Gems at Gelena (you can find this NPC near the Tree of Whispers).
Mind you, though, along with Fugitive Heads, you’ll also need the Restless Rot resource and Gem Fragments to craft an Occult gem.
