The primary way to obtain the Infernal Homunculus is by defeating the endgame lair boss Urivar, located at the Fields of Judgement. Defeating Urivar grants access to the boss chest, which requires a Lair Key to open. One creator's run showed defeating the Tormented Echo of Urivar at the Fields of Judgement in Nahantu on Torment 1, requiring character level 70 and 1 Lair Key at the Burned Altar to open Urivar's Hoard.
